While the city\u2019s vaccination coupons will only be sent to its residents, the following categories of people can call the city\u2019s vaccination center (092-260-8405) and book their vaccinations with coupon numbers provided by the municipalities in which they live:<\/span><\/p>\n

A – Pregnants temporarily living in Fukuoka City for childbirth\/ Students living in Fukuoka City while remaining registered residents of other municipalities\/ Workers who are temporarily living away from their registered residing municipalities.<\/span>
\n>>Submission of documents (<\/span>notification of vaccination outside registered residential municipality\/ \u4f4f\u6240\u5730\u5916\u63a5\u7a2e\u5c4a<\/span><\/a>) to Fukuoka City is required after receiving vaccination coupons. A certificate of submitting the document (\u4f4f\u6240\u5730\u5916\u63a5\u7a2e\u5c4a\u51fa\u6e08\u8a3c) would be issued before making a vaccination booking.\u00a0<\/span><\/p>\n

B – Hospitalized patients in Fukuoka City\/ Residents of Fukuoka City-based elderly care home and related facilities\/ Patients who treat their primary illness at Fukuoka City-based medical clinics and hospitals<\/span>
\n>>No document submission is required. Call the vaccination center with your vaccination coupons to make a booking.<\/span><\/p>\n

*In both cases, booking can be made ONLY AFTER receiving vaccination coupons from residing municipalities. Some municipalities begin distributing the coupons later than in Fukuoka City.<\/span><\/p>\n
