Who can apply for the program?<\/strong><\/h3>\n

Those whose residential address was\/will be in Fukuoka City at the time of their vaccination.
\nThose who have plans to visit countries and regions where some exceptions from quarantine and hotel stopover will be made for travelers with proof of COVID-19 vaccination.<\/p>\n

Normally, your \u2018Vaccine Passport\u2019 will be mailed out within 7 ~ 10 days after application.
\nHowever, office staff might call you in case some errors are found in submitted documents. Therefore, make sure your submitted telephone number is valid and useful.<\/p>\n
