Quick Guide To Obtain Your ‘Vaccine Passport’ in Fukuoka City

Fukuoka City began accepting applications for so-called ‘Vaccine Passports’ as of Jul. 16, and here are things you might want to know. (This is our summary and translation from the city’s website. This is for your convenience and not an official document – we hope you find it useful.

Who can apply for the program?

Those whose residential address was/will be in Fukuoka City at the time of their vaccination.
Those who have plans to visit countries and regions where some exceptions from quarantine and hotel stopover will be made for travelers with proof of COVID-19 vaccination.

*For domestic travels and other uses, ‘The new coronavirus Vaccine Injection Proof’ (新型コロナウイルスワクチン予防接種済証/新型コロナワクチン接種記録書) that is attached with their vaccination vouchers should do. Mail applications are required upon re-issuing the slip or an English version.

How to apply?

Travelers can apply either online or via mail. Make sure you have a passport that will be valid up to and during your travel period, as that passport number will be printed on the document.

Make sure you have following items before you begin;
• Passport
• Proof of vaccination (‘新型コロナウイルスワクチン予防接種済証’ or ‘新型コロナワクチン接種記録書’) attached with your vaccination voucher
• Personal ID (drivers license, health insurance card, etc.) to verify your mailing address
Access the website and enter required information, then take photos of the listed items and submit.

Prepare photocopies of required items and fill out the application form;
• Photocopy of your passport (the page including your passport number)
• Photocopy of proof of vaccination (‘新型コロナウイルスワクチン予防接種済証’ or ‘新型コロナワクチン接種記録書’) attached with your vaccination voucher
• Photocopy of personal ID (drivers license, health insurance card, etc.) to verify your mailing address
• Application form (excel / pdf, sample)

*DO NOT hand in these documents – there is no registration counter in the office (to prevent the spread of C19).

What will happen after application?

Normally, your ‘Vaccine Passport’ will be mailed out within 7 ~ 10 days after application.
However, office staff might call you in case some errors are found in submitted documents. Therefore, make sure your submitted telephone number is valid and useful.
